bug: CustomTemplateAPI in encounter form is breaking with error
Describe the bug
I'm probably the one that broke it so will get in and fix this, but looks like the CustomTemplateAPI is throwing errors when you use the nation notes functionality to insert items into the textbox. Need to figure out what is missing and fix it.
To Reproduce
Steps to reproduce the behavior:
- Add an encounter
- Double click on the reason for visit. Attempt to add a patient details and see the javascript errors, nothing is inserted
- Scroll down to '....'
- See error
Expected behavior
adding a template component should insert into the nation notes textbox.

- Browser: Chrome
- OpenEMR version: 7.0.3 in master
- Operating system: Ubuntu
