bug: Add Issue via Popups->Issues regression

Describe the bug

The dialog for adding an issue via Popups->Issues no longer displays Issue Types for selection, and there is no type selected by default, so the enhancement of "Hide/Show Fields" is also broken. Reference PR

To Reproduce

Select a patient.
Select "Popups->Issues".
Select "Add Issue".
Check the "Type" radio list.
