The window text and icon can't be vertically centered because the titlebar height is 19px and the icons are size 16x16, so it will always be one pixel closer either to the top or bottom.
The two most logical solutions would be to use 18px or 20px tall titlebars.
I made some mockups so you can see it better. Please disregard the window control buttons for now.
